Description
As an Administrative Assistant supporting the Clinical Time Transition Program (CTTP) you will provide essential support to a diverse treatment team working with youth ages 11-17 as they transition from a hospital setting back to their community. You will handle client information keeping with standards of confidentiality. You will report to the Director of the Hospital to Home (H2H) Program, and you will work onsite with occasional travel for meetings or community events.

Responsibilities
- Manage scheduling for team meetings, client appointments, and follow-up sessions
- Manage case files, records, and documentation in compliance with confidentiality regulations (HIPAA)
- Prepare reports, correspondence, and meeting materials
- Help with data entry and maintain electronic health records (EHR)
- Be a primary contact for families, community partners, and providers
- Answer and direct phone calls, emails, and other inquiries
- Coordinate referrals, intake paperwork, and other documentation needed for client transitions
- Help with communication between team members to ensure continuity of care
- Track deadlines for treatment plans, assessments, and required documentation
- Order and maintain office supplies, forms, and resource materials
- Arrange transportation or other logistical needs for clients
- Support the team with event planning, training sessions, and outreach activities
- Ensure compliance with organizational policies, procedures, and regulatory requirements
- Maintain confidentiality and security of client records and information
- Help monitor documentation accuracy and completeness for audits and reviews
- Handle sensitive situations with empathy
- High school diploma or equivalent required; associate’s or bachelor’s degree preferred
- 2+ years of administrative experience in a healthcare or social services setting required
- Proficiency in electronic health records (EHR) required
- Experience working with youth or families in crisis required
