Key Responsibilities
- Build trusting relationships with members by providing guidance and support throughout their healthcare journey.
- Coordinate care by collaborating with healthcare providers, pharmacies, and community organizations to ensure comprehensive service delivery.
- Conduct health assessments and develop personalized care plans tailored to each member’s needs.
- Identify and address Social Determinants of Health (SDOH) that may impact overall member well-being.
- Communicate with members via phone and text to educate them on program benefits and assist with appointment scheduling.
- Maintain meticulous records within the care management platform, ensuring adherence to best practices.
- A minimum of 2 years of experience in care coordination, case management, community health, or a comparable field.
- Excellent communication skills, both verbal and written, coupled with strong problem-solving capabilities.
- Proficiency in utilizing Apple computers (e.g., MacBook) and experience in electronic health records platforms.
- Familiarity with motivational interviewing techniques to empower members effectively.
- Fluency in English is required, with Spanish language skills being highly preferred.
- Candidates must reside in Tennessee.
- LPN or RN designation preferred.
