Standard work schedule: Monday-Friday 8:00am 5:00pm

Hourly Pay Rate: $17

Who We Need

The Re-Engagement Navigator (Peer Navigator) demonstrates the possibility of recovery through sharing of personal experiences to inspire hope and empower people to succeed in their personal goals. This position works collaboratively with an assigned caseload and with other members of the team to complete re-engagement efforts for the assigned clinic. The Re-Engagement Navigator works under the direction of the site Lead Clinical Coordinator following the established 8-week protocol as directed through policy. This position demonstrates a strength-based approach in interacting with people by fostering skills in self-management by drawing on the individual’s personal strengths and natural supports, and empowers the person to be the leader of their treatment and care.

MINIMUM QUALIFICATIONS:

  • Must be 21 years old or older with a high school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health OR
  • Bachelor’s degree in behavioral health related field; or
  • Bachelor’s degree in non-behavioral health related field and at least one (1) year of full-time direct care
  • experience in behavioral health; or
  • Associate degree in behavioral health related field and two (2) years of full-time direct care experience in behavioral health experience.
  • Peer Support Certification highly preferred.
  • Bilingual (English/Spanish) preferred.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB Test clearance required at the time of hire.

Job Summary (including, But Not Limited To)

  • Participates in outreach, engagement, and re-engagement with a focus on community-based re-engagement, up to and including frequently meeting with members in the community (e.g., hospitals, jails, homes, shelters etc). Follows the outreach workflow established for re-engaging members who have disengaged from treatment.
  • Works with all staff in the assigned clinic to ensure communication and coordination of re-engaging these members as a shared responsibility with each team.
  • Plans, prepares, and delivers services according to protocol and adjusts schedule to the caseload needs
  • Identifies, analyzes, and solves problems in the completion of tasks using critical thinking and problem-solving skills
  • Participates in morning meetings, staffing's, and other assigned meetings for assigned members, and reports to Clinical Coordinator regularly of any issues or concerns in a timely and professional matter. Follows up on all communication and assigned tasks.
  • Establishes relationships with community organizations to locate members (CASS, hospitals, jail, shelters), case managers, teams, staff, etc. to ensure timely completion of tasks weekly.
  • Identifies and tracks daily tasks and key challenges in locating members and communicates these to Clinical Coordinator weekly.
  • Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols.
  • Adherence to all agency policies, licensure, and training requirements.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
