Position Summary: Under the supervision of the Director Foster Care Mental Health, the Foster Care Mental Health Coordinator is responsible for coordinating and following up on behavioral health referrals for foster care children. The FCMH Coordinator works collaboratively with the different care teams to ensure that children are receiving the necessary services and assists in coordinating services as needed.

Essential Duties:

  • Understand and promote the organization’s Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Review referrals for mental health services, determine which evaluations are needed at the time of referral.
  • Coordinate appointments for evaluations with families and inform case planning teams of appointments.
  • Maintain up to date log of all referrals received and appointments scheduled, and ensure all referrals are scheduled in a timely manner.
  • All attempts to communicate with families will be documented in ECW.
  • Review evaluation reports for recommendations, follow up and coordinate any mental health-related recommendations (further evaluations, therapy, etc.).
  • Distribute evaluation reports to the foster care planning teams.
  • Scan any reports received from the foster care teams (referral forms, IEP, discharge summaries, prior evaluations, etc.).
  • Assist mental health providers with scanning/uploading client documentation into electronic health records.
  • Assist with requesting relevant client documentation from hospitals, outside providers, referral sources (evaluations, discharge summaries, etc.).
  • Referrals/Recommendations for therapy are added to FCMH therapy waitlist, and communicated to supervisor.
  • Ensure consents are in ECW for all evaluations and therapy before coordinating appointments, document all attempts to obtain these documents.
  • Ensure all evaluation appointments are confirmed and rescheduled as needed.
  • Document all information on ECW regarding evaluations, appointments made, referrals, communication with families/care teams.
  • Communicate any medication-related concerns to appropriate staff member and supervisor.
  • Communicate with appropriate staff member when referral for outside community mental health provider is needed.
  • Assist with translation as needed for psychiatrist and psychologists.
  • Communicate any immediate mental health service needs or clinical concerns to supervisor.
  • Attend foster care, mental health and administrative assistant meetings as needed.
  • Assist with audit preparations.
  • Provide on-site coverage for mental health providers in Bronx Health Services Clinic and Harlem Family Services Center.
  • Comply with all privacy laws including HIPAA, etc.
  • Perform other duties as assigned.

Minimum Qualifications:

  • Associates Degree Required
  • Bilingual (Spanish/English) required.

Key Competencies:

  • 3 years’ experience with coordinating mental health services preferred.
  • Excellent time management skills.
  • Strong interpersonal skills.
  • Ability to work independently.
  • Ability to work collaboratively and effectively with others.
