Administrative functions:

  • Assist in the collection of resident assessment data (MDS), in accordance with current rules, regulations, and guidelines, that govern the resident assessment, including the implementation of CAA and triggers.
  • Review updates of written policies and procedures that govern the development, use and implementation of the resident assessment ( MDS) and care plan
  • Participating in, an ongoing quality assurance program for the collection of resident data of resident care plans
  • Participate in functions involving discharge plans as may be necessary
  • Implements procedures with the RN resident assessment/care plan coordinator to inform all assessment team members of the arrival of newly admitted patients
  • Develop the schedule of activities required for completion of the RAI process
  • Assist with coordinating the development of a written plan of care ( preliminary and comprehensive) for each resident that identifies that problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Conduct daily, concurrent utilization review via chart audit in order to ensure that change of condition RAI’s are initiated when appropriate
  • Conduct day concurrent utilization review via chart audits in order to ensure that discharge planning proceeds smoothly relative to resident’s progress and Medicare coverage status.
  • Main the confidentiality of all resident care information


Qualifications & Experience Requirements for MDS Coordinator:

  • Previous Experience as a MDS Coordinator preferred
  • Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Connecticut required
  • Must be an LPN or RN
