Position Summary
As a member of the Care Team, the LMFT Behavioral Health Specialist is a core member of the collaborative care team. This includes the patient’s medical provider and psychiatric consultant, as well as the larger primary care or medical team. The LMFT Behavioral Health Specialist is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider, and, when appropriate, other mental health providers. In addition, the LMFT Behavioral Health Specialist must be able to assist the patient in a sensitive and supportive manner, while acting as an advocate on behalf of the patient.

Essential Functions And Responsibilities Of The Job
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Ability to maintain effective and professional relationships with patients and other members of the care team.
- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.
- Ability to screen for common mental health and/or substance abuse disorders.
- Effectively engage patients in a therapeutic relationship, when appropriate and thru assessment and treatment planning for common mental health and/or substance use disorders.
- Utilize evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation and/or counseling techniques).
- Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice.
- Contributes effectively to the overall function and organization of the Medical Foundation provider clinics and centralized Medical Management services.
- Monitor and implement all regulatory changes impacting care.

- Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Qualifications/Work Experience:
- Previous experience (min 3 years) functioning as a clinical social worker or marriage and family therapist with an emphasis on behavioral health.
- General knowledge of health care delivery systems and regulatory requirements.
- Proficiency in computer and software utilization in support of job functions.
- Bilingual Spanish/English preferred
- Licensed Marriage and Family Therapist (LMFT) in the State of California
