The Case Manager at A New Leaf’s Faith House Emergency Shelter provides compassionate support and guidance to individuals and families escaping domestic violence. This role involves coordinating essential services, developing individualized service plans, and empowering clients to achieve stability and self-sufficiency. The Case Manager serves as a vital advocate, connecting clients with resources such as housing, counseling, legal assistance, and employment opportunities.

Qualifications

Education:

  • High School or GED required.
  • Associate or Bachelor’s Degree, preferred.

Experience

  • Experience working with domestic violence, homeless or economically disadvantaged population required.
  • Knowledge of community resources.
  • Knowledge of and/or experience with trauma informed practices required.
  • Bilingual verbally in Spanish, preferred.

Duties And Responsibilities

  • Role model appropriate behaviors to callers/clients in the shelter, at all times, utilizing trauma informed principles and practices. 
  • Provide case management, crisis intervention, domestic violence education, and resources and referral services to callers on crisis calls and to clients in the shelter. 
  • Communicate with hotel/motel and/or transportation agencies for client placement in overflow program and convey information appropriately to internal programs. 
  • Perform screenings, lethality assessments, and basic needs assessments for potential clients according to procedure. Assist with the introduction of program and facility orientation to incoming clients. Assist with client intake/exit paperwork as needed
  • Co-develop 24/7 Safety/Crisis Plans with every household, depending on the safety needs and risk factors identified by the client. Monitor the implementation of a strength-based service plan and progress of household towards goals.
  • Assist households in obtaining community services and benefits, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies/services.
  • Provide referrals and/or coordinating transportation for households to community services as relevant to client need. Facilitate and coordinate discharge planning transition and follow up according to plan.
  • Recognize common symptoms of mental health related issues and/or substance use disorder, including common side effects, reactions/interactions of medications prescribed.
  • Maintain ability to drive agency passenger van to transport clients and complete other duties, as needed and/or drive personal vehicle to conduct program business (mail runs, grocery shopping, etc.).
  • Complete accurate documentation in database and all program paperwork in a timely, correct, and thorough manner. Be able to lift a minimum of 30lbs
  • Facilitate psychoeducational groups for clients based on topics relevant to client needs. Assist in the translation of barriers and successes into positive system changes (as relevant to program/client needs).

  • Class One Fingerprint Clearance Card
  • Valid Driver’s License with reliable transportation, required
  • Current Auto Liability Insurance, required
  • CLEAN 39 Month Motor Vehicle Record, required
  • Be at least 21-years of age
