Wellness Coach

Triboro Center is looking to hire a Wellness Coach to work Part-Time in Bronx, NY.

Key Responsibilities:

• Develop and implement individualized wellness programs tailored to residents’ physical abilities, cognitive levels, and personal interests.
• Conduct regular wellness assessments, monitoring progress, and adjusting programs as needed.
• Lead group activities such as light exercise, mindfulness, stress-reduction, and wellness workshops.
• Partner with rehabilitation, nursing, and dietary teams to promote healthy routines and continuity of care.
• Provide motivational coaching and support to residents in areas such as mobility, nutrition awareness, social engagement, and emotional resilience.
• Educate residents and families on wellness practices to support independence and overall health.
• Collaborate with recreation staff to design engaging group activities that promote physical movement, social connection, and mental stimulation.
• Maintain accurate records of resident participation, progress, and feedback.
• Support a culture of holistic care in line with Triboro Center’s mission to enhance quality of life.
  
  
  

Qualifications:

• Bachelor’s degree in Social Work, or related field (preferred CASAC).
• Certification in substance abuse and mental health certification
• Previous experience working with seniors, especially in long-term care and or drug rehabilitation.
• Strong communication, motivational, and interpersonal skills.
• Ability to adapt wellness programs to accommodate a wide range of physical and cognitive needs.
• Compassionate, patient, and enthusiastic about improving residents’ lives.
