  • Documents care, monitors patient progress, and communicates updates with families and healthcare teams.
  • Administers medications, offers health education, and conducts assessments to support independence and disease management.
  • Supervises caregivers, ensures compliance with policies and regulations, and manages medication protocols.
  • Completes necessary documentation, participates in staff training, and may handle case management and after-hours responsibilities.
  • Requires a nursing degree, licensing, CPR certification, and preferably home health experience. Strong organizational, communication, and clinical skills are essential.
