The Population Health Nurse will report to Supervisor, Value Base Reimbursement and is responsible for improving the overall health outcomes of defined populations by analyzing health data, developing and implementing health programs, and collaborating with community stakeholders. This role focuses on assessing patient needs, coordinating services, medication adherence and patient education.

This role focuses on assessing patient needs, coordinating services, medication adherence and patient education.
Responsibilities
- Perform outreach to patients who have been discharged from the ER or an inpatient stay.
- Provide education and training to patients and their caregivers on managing health conditions, medication adherence and self -care techniques.
- Assist patients in accessing necessary medical services, treatments, and community services.
- Monitor and analyze patient data to track outcomes, identify trends, and measure the effectiveness of care interventions.
- Identify and report barriers to care and follow-up with care team.
- Maintain accurate and up-to-date documentation of patient care, outcomes and quality metrics.
- Work collaboratively with an interdisciplinary team, including physicians, advance practitioners, social workers and other healthcare professionals
- Contribute to quality improvement initiatives and program development to enhance care delivery and patient outcomes.
- Stay informed about best practices and incorporate new knowledge into practice
- Bachelor’s degree in nursing (BSN) required.
- Registered Nurse licensed in both AZ and NV
- Minimum of 5 years’ experience in public health, community health, care coordination, case management or related nursing field
- Knowledge of WORD, EXCEL, POWERPOINT and Epic
- Bilingual English/Spanish
- Experienced in Value base care
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently and as a part of a collaborative team, demonstrating professionalism, reliability, and a positive attitude.
- Ability to manage multiple tasks and priorities in a fast-paced environment.
- Proficiency in using the electronic health record (EHR) and other healthcare management software. Epic experience preferred
- Knowledge of healthcare systems, insurance policies and community resources.
- Ability to handle and maintain extreme confidentially with patient records.
- Ability to develop and maintain collaborative and relationships effective working relationships with management and all levels of staff.
- Ability to maintain and appropriate professional appearance and demeanor.
