We are looking for passionate professionals eager to make a difference in the community through personalized care and innovative treatment plans.
Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans.
- Deliver evidence-based therapeutic interventions to improve mobility and function.
- Track patient progress and adjust treatment plans as needed.
- Collaborate with interdisciplinary healthcare teams to ensure holistic care.
- Educate patients and caregivers on therapy goals and home exercise programs.
- Maintain accurate and timely clinical documentation.
- Participate in continuing education and professional development activities.
- Active Florida Physical Therapist License (Required)
- Experience in home health or outpatient settings preferred
- Strong interpersonal and communication skills
- Self-motivated and able to work independently
- Passion for delivering high-quality, compassionate care
- Flexibility to work PRN hours as needed.
