Meriden Healthcare & Rehab is now hiring a Physical Therapist (PT) to provide rehabilitation services to our patients and residents. This facility is seeking a caring, warm-hearted Physical Therapist who is currently licensed in Connecticut or is a graduate from an approved Physical Therapist program and is actively working toward obtaining licensure. Be a part of our in-house team of compassionate rehabilitation professionals who are committed to helping each patient recover the strengths, skills, independence, and confidence to return to their highest level of function, and an active life. Responsibilities:
- Develop and implement physical therapy programs that focus on rehabilitation per state practice act
- Work directly with patients to achieve maximum physical recovery
- Evaluate effects of therapy treatment and communicate patients' progress
- Record and document patient care services accurately and concisely
- Collaborate with other team personnel to achieve well-rounded care
- Graduate of an accredited Physical Therapy Program.
- Current CT State PT License.
- Experience in SNF settings is preferred, but not required.
- Exceptional customer service and patient care experience.
- Ability to work in a fast-paced environment while maintaining a positive attitude.
