Qualifications

Qualifications

  • All Level I, II, III, and IV competencies.

Education

  • HS diploma or GED required.

Experience

  • At least 48 months (36 months at implementation) hospital pharmacy (or equivalent as determined by pharmacy leadership), Level III for at least 1 year, full time, exceeds the minimum requirements of Level I, II, and III.

Licensure/Certification/Registration/ETC.

  • OK permit, PTCB, CPhT certification, and Advanced Certified Pharmacy Technician certification required
