The Performance Excellence Director is responsible for leading enterprise-wide initiatives focused on quality improvement, change management, Lean Healthcare transformation, risk mitigation, regulatory compliance, clinical excellence, and patient experience. This role provides direction to enhance patient outcomes, operational efficiency, patient-centered care, and organizational performance while fostering a culture of continuous improvement. This role collaborates with executive leadership, clinical teams, and operational stakeholders to align quality and performance.

Accountabilities Include
Performs Essential Job Duties
- Works in collaboration with the Chief Quality Officer to facilitate the development of the annual Performance Excellence Plan with service area leaders.
- Implements a performance measurement framework, ensuring alignment with strategic goals, patient experience metrics, and regulatory requirements.
- Ensures appropriate support for quality improvement (QI) projects to enhance clinical workflows, reduce inefficiencies, and improve patient outcomes.
- Ensure organizational alignment with value-based care models, population health initiatives, and care coordination programs to enhance health equity and improve community health outcomes.
- Utilizes data-driven decision-making by developing key performance indicators (KPIs) and leveraging analytics tools to monitor trends, patient satisfaction, and care outcomes.
- Ensures compliance with regulatory agencies related to federally qualified health center services. (HRSA, DMS, NCQA, Patient Centered Primary Care Home and other)
- Ensures and oversees culture of safety initiatives and responsible event reporting systems facilitating the development and completion of corrective action plans as needed.
- Develops strategies to reduce clinical variation, enhance care coordination, and address disparities in health outcomes for underserved communities.
- Ensures implementation of systems for policies and procedures.
- Drives patient experience initiatives to ensure high-performance in patient satisfaction and a patient centered approach across all service lines.
- Oversees patient feedback mechanisms, such as patient satisfaction surveys and advisory councils, to ensure the voice of the patient is reflected in quality improvement efforts.
- Ensures training programs are in place and implemented on patient centered communication, cultural competency and trauma informed care to enhance the patient experience.
- Develops and oversee a risk mitigation strategy that addresses patient safety, operational vulnerabilities, and regulatory compliance.
- Ensures high-level best practice assessments, root cause analyses (RCA), and Failure Mode and Effects Analyses (FMEA) to identify and address potential failures.
- Serves as the organizational Privacy Officer.
- Provides support for leadership in ensuring compliance with regulatory and accreditation standards (federal, state, OSHA, PCMH, NCQA, etc.).

- Bachelor’s degree in related field
- Require Lean Health Care/Six Sigma Certification or CPHQ
- Three-five years’ experience in healthcare management, clinical profession, or related field
- One year’s experience in process improvement or quality related activities.
- Three-five years of leadership or management experience.
- Experience in regulatory compliance and/or accreditation processes.
- Knowledge of statistics, data collection, analysis, and data presentation
- Comfort in teaching workforce, leadership, and board.
- Ability to work independently and is self-directed.
- Experience in project management
