In this pivotal role, you will collaborate closely with the Program Manager to optimize both fiscal and programmatic functions, ensuring the effective coordination and delivery of essential services to the veterans we serve.
Key Responsibilities
- Conduct engaging training sessions for veterans and their representatives.
- Perform quarterly home visits for assigned veterans to assess and address their needs.
- Manage the intake process for new VD-HCBS referrals, including submission of enrollment documents to financial management services and the Department of Veterans Affairs.
- Maintain comprehensive documentation of all correspondence related to VD-HCBS referrals and veteran files.
- Review and ensure accuracy of veterans’ timesheets with necessary corrections in the electronic database.
- Develop and evaluate veterans' budget plans for precise financial reporting.
- Assess veterans’ needs and verify appropriateness and effectiveness of services provided.
- Formulate improvement or corrective action plans as required.
- Communicate effectively with the Program Manager and financial services regarding any changes in a veteran's institutional placement.
- Collaborate on new enrollments, changes in veteran status, and issue resolution with the Program Manager.
- Address veteran-related concerns with the Program Manager and assist in the approval of necessary paperwork.
- Liaise with financial management services regarding enrollments and budget plan adjustments.
- Provide ad hoc reports as necessary.
- Update and submit the VD-HCBS Monthly Update Spreadsheet by set deadlines.
- Conduct background checks for all employees selected by the veteran for hire.
- Maintain and update a summary page for each veteran as needed.
- Submit a detailed home visit/phone call report to the Program Manager on a monthly basis.
- Complete and submit mileage reports with appropriate documentation in a timely manner.
- Participate in meetings and conference calls with local and national VA representatives.
- Adhere to all security and confidentiality protocols.
- Undertake additional duties as assigned by the Program Manager or other leadership.
- Exceptional ability to prioritize tasks and adapt to changing circumstances.
- Strong customer service acumen, demonstrating professionalism and helpfulness.
- Collaborative and team-oriented attitude.
- Effective listening and information-gathering skills.
- Capable of independent work while managing multiple projects and priorities.
- Familiarity with federal and state-funded programs.
- Proficient in Microsoft Word and Excel.
- Strong verbal and written communication skills.
- Consistently meet deadlines.
- Must possess a valid driver’s license and be willing to travel to various locations.
- A minimum of 2 years of experience in case management is required.
- An Associate’s or Bachelor’s degree is preferred; however, relevant education combined with experience will also be considered.
