We’re seeking an experienced Occupational Therapist (OT) for a contract assignment in a Skilled Nursing Facility (SNF) located in Yuba City, California. This role offers guaranteed hours and the opportunity to make a meaningful impact on patient recovery and rehabilitation.
Position Details
- Location: Yuba City, CA
- Setting: Skilled Nursing Facility (SNF)
- Schedule: Monday – Friday, 6–8 hours per day
- Contract Length: 13 weeks
- Hours: Guaranteed 36 hours per week
- Productivity: 90% required
- Orientation: 4 hours (unbillable)
- Active Occupational Therapist License (California)
- BLS certification (required)
- Minimum 1 year of recent SNF or rehab experience preferred
- Must meet pre-employment credentialing requirements (background check, drug screen, etc.)
- Evaluate, plan, and implement individualized treatment programs for patients
- Assist patients in improving functional independence in daily living activities
- Collaborate with physicians, nurses, and rehab staff to coordinate patient care
- Document patient progress and adjust treatment plans as necessary
- Provide education to patients and families on rehabilitation techniques and home exercise programs
- Ensure compliance with facility policies, procedures, and state regulations
- Monday–Friday schedule with consistent hours
- Guaranteed 36 hours weekly
- 13-week contract assignment
