Nursing Comprehensive Assessment Specialist

RN Opportunity – Comprehensive Assessment Specialist in Nursing

Onsite across multiple centers in Brockton / Attleboro / Brighton, MA

Salary: $80, 000-85,000+ PLUS mileage reimbursement

Schedule: Monday–Friday | Day Shift | Full-Time (40 hours)

In this role, you’ll be the vital link between clinical care and quality improvement—ensuring assessments are accurate, timely, and person-centered.

What You’ll Do

• Lead the comprehensive assessment process, from scheduling to submission with regulatory agencies and funders.
• Oversee clinical information gathering to support individualized care plans.
• Review MARs, TARs, flow sheets, and physician orders for compliance.
• Conduct interviews and physical assessments for new referrals.
• Maintain accurate, timely entries into EMRs and follow through with funders.
• Facilitate care plan conferences with the interdisciplinary team and families.
• Generate Quality Improvement reports to spot trends and strengthen training.
• Ensure nursing compliance and support the Director of Quality Improvement.
  
  

What We’re Looking For

Graduate of an accredited School of Nursing with an active RN license.

At least 2 years of nursing experience (supervisory or MDS experience a plus!).

Strong communication and organizational skills.

Self-motivated, detail-oriented, and analytical.
