Position: Life Advancement Specialist – In-Home Support ServicesReports To: Associate Director of Residential Services / Director of Residential Services
Hours: Per diem, up to 24 hours per week
Overview
Triangle has an immediate opening for a Life Advancement Specialist in our In-Home Support Services. The qualified candidate will provide individualized, in-home support to adults with disabilities, empowering them to develop life skills, pursue personal goals, and integrate more fully into their communities. This role emphasizes person-centered planning, flexible scheduling, and close collaboration with the residential leadership team.

Responsibilities
Daily: Provide 1:1 in-home coaching tailored to each resident’s needs and goals; document and bill services; monitor engagement and safety; adjust the day’s activities as needed by the resident.
Weekly: Bill contracted hours; check in with supervisor; facilitate social outings; share progress and challenges.
Weekly/Biweekly: Check in with the Associate Director of Residential Services to review cases, schedules, and any emerging needs.
Monthly
Perform home environmental inspections and complete vehicle safety checklists; submit reports.
Meet with the Associate Director of Residential Services for supervision and case review.
Yearly: Meet department goals for Resident independence and community integration; participate in annual ISP/Life Plan and behavioral progress meetings.

Work Environment: Supports occur in private residences and community settings. Employees must adapt to varied environments and respect participants’ privacy; sensory inputs may vary.

Responsibilities
- Assessment & Planning: Conduct initial and ongoing assessments (performed by the Supervisor) to identify strengths, preferences, and life-goal objectives; implement personalized support plans developed in collaboration with the Supervisor to ensure resident-focused outcomes.
- Skill Building & Coaching: Teach and reinforce independent‑living skills (e.g., budgeting, personal hygiene, cooking, household management); facilitate self‑advocacy and communication.
- Companionship: Provide social interaction, emotional support, and engage in activities that promote well‑being.
- Medication Management: Assist with medication reminders and ensure timely, correct administration, and provide additional support as needed, including administration under guidance based on residents’ individual plans.
- Household Tasks: Assist with cleaning, laundry, and grocery shopping—overseeing each task and coaching residents through hands-on practice to build their skills.
- Meal Preparation:Assist with meal planning and cooking, offering oversight and hands-on coaching to develop residents’ meal-preparation skills.
- Transportation: Support travel to medical appointments, community outings, and social activities.
- Communication & Advocacy: Help individuals communicate their needs and advocate for their rights with families, providers, and community agencies.
- Care Coordination: Collaborate with healthcare, residential, day program, employer and family stakeholders to maintain seamless, person‑centered support.
- Emergency Preparedness: Guide residents in developing and implementing in‑home disaster and emergency plans.
- Community Integration: Accompany and coach residents in community settings; establish connections with local resources and social groups.
- Safety & Compliance: Ensure residents safety and well‑being in their home environment; adhere to agency policies and safety protocols.
- Documentation & Reporting: Maintain accurate, timely notes in Penelope; submit daily billing and progress notes; complete and submit a monthly environmental inspection report and vehicle checklist to the residential site supervisor; and perform any additional documentation as necessary to support resident care.
- Supervision & Collaboration: Participate in team and ISP/Life Plan meetings; meet biweekly and monthly with the Associate Director of Residential Services for case review and supervision.
- Other duties as assigned/needed.

Qualifications
- Education & Experience: High school diploma or equivalent required; Associate’s or bachelor’s in human services, Social Work, Education, or related field preferred. Minimum of 1 year supporting adults with disabilities in home or community settings.
- Skills & Certifications: Strong interpersonal/coaching skills; excellent written and verbal communication; valid MA driver’s license and clean driving record; reliable personal vehicle; ability to pass state and federal background checks; CPR/First Aid preferred or willingness to obtain. Safety Care training will be required.
- Preferred Licenses & Certifications: MAP certification preferred, or ability to obtain within 90 days of hire.
- Attributes: Person-centered approach, patience, cultural sensitivity; flexible availability for evenings and weekends (per diem); commitment to promoting resident independence, dignity, and quality of life.
