As part of a statewide initiative in collaboration with a leading national partner, this role plays a key part in ensuring adults with IDD receive the services they need.
Location: Assigned Region: Queens (90% Remote, 10% In-Field)

- Schedule and conduct in-person assessments in home and community settings while following a hybrid work model.
- Travel within Queens Region to meet with individuals, families, and caregivers.
- Use a person-centered interview approach to assess individuals' needs and strengths.
- Ensure accuracy and completeness of assessments in compliance with state guidelines.
- Interview caregivers, providers, and other key stakeholders to gather necessary information.
- Utilize multiple data sources, including records review, for assessment coding.
- Enter data into assessment software and meet documentation deadlines.
- Maintain productivity and quality standards as required.
- Arrange interpreter services as needed for languages other than English and Spanish.
Education
- Required: Bachelor’s degree in a human services field.
- Preferred: Master’s degree in a human services field.
- Minimum one (1) year of direct experience working with adults with intellectual and/or developmental disabilities (IDD).
- Preferred: Experience conducting assessments and prior CAS experience.
- Strong analytical and problem-solving skills.
- Ability to work independently while meeting deadlines.
- Excellent written, verbal communication, and presentation skills.
- Proficiency in Microsoft Word, email, and web-based applications.
- Bilingual (English/Spanish) – Preferred.
- Must possess a valid NYS Driver’s License or NYS ID card.
- Must be willing to travel throughout assigned location.
