We are excited to announce an opening for a Registered Nurse (RN) to join our compassionate team! In this vital role, you will be responsible for enhancing the quality of life for our clients by providing essential assessments, developing care plans, and ensuring excellent communication among caregivers and clients. Your expertise will help keep our clients safe and independent in their homes. Our mission is simple: improve more lives!

What You’ll Do

  • Be the First Point of Contact: Provide excellent customer service to clients (current and potential), industry partners, and care providers.
  • Schedule Assessments: Perform in-home assessments and re-assessments for clients.
  • Write Care Plans: Ensure Caregiver Managers have the necessary information to review with caregivers before care begins.
  • Assess Client Needs: Admit new or current clients for nurse delegation, medication, or other specialty care in accordance with state law.
  • Conduct Visits: Complete caregiver observation and client supervisory visits, documenting in client and caregiver files.
  • Update Care Plans: Communicate any changes to caregivers, route updates to clients for review and signature, and document accordingly.
  • Provide Training: Perform caregiver training and delegate additional training as needed.

Work Schedule

  • Hours: 40 hours per week
  • Typical Office Hours: Monday – Friday, 8 AM - 5 PM. Occasional evenings and weekends may be required.
  • Location: 60% of your time will be in an office environment; 40% will be in various client homes for assessments or observations.

Are You a Good Fit?

If you’re dedicated to improving lives, you’re on the right path! Our ideal candidate has:

  • Active Oregon RN Licensure
  • Some Senior Care Experience (preferred)
  • Strong Communication Skills: Attention to detail is key!

  • Valid driver’s license, auto insurance, and a clean driving record
