Description

The Care Management Associate role works under the direction of Care Managers and Care Management Social Workers to help coordinate payer communication, implement the transition plan, deliver patient documentation and other duties in support of care management departmental functions.

Required Minimum Knowledge, Skills, And Abilities (KSAs)

  • Education: Associate’s Degree or three years of medical experience preferred.
  • License/Certifications: N/A
  • Experience: One year of experience in a healthcare setting preferred. Experience with community resources or insurance companies preferred.
  • Additional Skills/Requirements Required: N/A
