Overview

The Weitzman Institute is a national leader in evidence-based, stakeholder-informed primary care innovation and transformation, serving over 2 million patients across all 50 U.S. states, DC, Puerto Rico, and Guam through partnerships with FQHCs and other health systems. Established in 2007 as the first research, education, and policy center embedded within a large safety-net primary care health system, we ground our efforts in the real-world experiences of patients and healthcare teams, ensuring our work is both practical and impactful. Its unique environment, within a safety-net health system, has enhanced its ability to conduct research and promote innovation aimed at improving outcomes of vulnerable populations and developing implementation strategies for evidence-based, integrated practice for Federally qualified health centers (FQHCs) and other safety-net, primary care practices nationwide.

The Weitzman Institute seeks an Analyst/Patient and Community Liaison who will serve as a bridge between data and lived experience. This role combines analytical skills with deep community engagement to inform and improve patient-centered initiatives, especially for medically underserved populations. The ideal candidate will analyze health-related data to identify trends and differences in outcomes, while also engaging patients and community members to ensure their voices shape care delivery, research, and quality improvement efforts.

This position will require frequent travel across six cities/towns in Connecticut: Bristol, Hartford, Meriden, Middletown, New Britain, and Waterbury.

Minimum Qualifications

Education/Training

Bachelor’s degree in public health, social work, health sciences, or related field (Master’s preferred).

Experience

  • 3+ years of experience in community engagement or health data analysis.
  • Demonstrated ability to work with diverse populations, especially those historically underserved or marginalized.
  • Strong written and verbal communication skills.
  • Experience with data tools (e.g., Excel, Tableau, or SPSS/Stata/R) and/or qualitative analysis (e.g., NVivo, Dedoose).
  • Familiarity with patient-centered outcomes research (PCOR) or community-based participatory research (CBPR).
  • Prior experience in healthcare, FQHCs, or nonprofit settings.
  • Ability to work independently and with others, particularly in a remote-based work environment.
  • Interpersonal skills to work with all levels of administrative and clinical staff, as well as external stakeholders.
  • Capacity to handle competing priorities, and make timely, sound decisions under pressure.

Language

  • Must be proficient in Spanish

Primary Duties And Responsibilities

  • Patient & Community Engagement
    • Serve as a trusted liaison to patients, families, and community partners, ensuring their feedback is represented in institutional decision-making.
    • Conduct outreach, interviews, and focus groups with patients and community members.
    • Facilitate and support community advisory boards, patient panels, and town halls.
    • Build relationships with local organizations and community leaders to foster collaboration and trust.

  • Data & Program Analysis
    • Collect, analyze, and interpret quantitative and qualitative data related to patient outcomes and program performance.
    • Generate actionable insights and contribute to reports, presentations, and dashboards for internal and external stakeholders.
    • Support evaluation of community-based programs and patient engagement strategies.
    • Collaborate with policy and research teams to incorporate community-informed indicators in data analysis.

  • Capacity Building & Communication
    • Translate research and data findings into plain language for community dissemination.
    • Support training sessions for patients and community members on topics such as health literacy, research participation, and advocacy.
    • Co-develop culturally appropriate materials and outreach strategies with patients and community members.
    • Performs other duties as assigned.
