Position Overview
The Aftercare Success Coordinator builds consistent, responsive relationships with youth and young adults navigating instability, offering hands-on mentorship, resource navigation, and individualized support. This role helps participants stabilize and reengage at any point in their journey, whether exiting a CHOOSE 180 cohort, incarceration, or surviving community violence. Aftercare Success Coordinators, using the Support Assessment Model (SAM), tailor case management, guide referrals, and ensure every young person receives presence, connection and stabilization.
Aftercare Success Coordinators carry a caseload of approximately 15-20 active youth, with engagement varying based on each young person’s SAM tier and goals. Support may range from light-touch encouragement to intensive, daily stabilization efforts.

Key Responsibilities
Case Management and Stabilization Support
- Build consistent, trust-based relationships with youth navigating instability, school pushout, violence, and system involvement
- Use the SAM framework to assess needs and adapt support in real time, grounded in trauma-informed practice
- Co-create individualized Youth Action Plans that support healing, address urgent needs, and guide re-engagement that include but are not limited to securing documentation such as social security cards, I.D., working with youth to identify transportation to and from interviews, or court ordered meetings
- Provide 1:1 support through but not limited to weekly in-person and virtual check-ins, transportation, and safety planning
- Support participants in reflecting on their strengths and growth points
- Document progress notes, assessments, calendars, and reports in Apricot and other CHOOSE 180 systems
- Facilitate Aftercare Curriculum modules and at times, drop-in group spaces that focus on decision making, emotional regulation, employment, and reengagement
- Integrate CHOOSE 180’s COMPASS curriculum tools to build confidence and activate youth voice
- Highlight milestones and in collaboration with youth, identify steps towards long-term goals
- Support youth in navigating schools courts, housing, employment, and mental health systems
- Identify and if applicable, connect youth to CHOOSE 180 workshops, counseling groups, employment, internships, and partner programs.
- Attend reentry, IEP or court meetings as needed to advocate and ensure alignment with youth goals
- Coordinate with CHOOSE 180 teams and external partners to ensure continuity of care and prevent youth from setbacks
- Advocate for participants within housing, education, and workforce systems, focusing on equity and opportunity.
- Participate in CHOOSE 18O all-staff, program team, and training meetings.
- Support community outreach, weekend events, and other organization-wide responsibilities as needed
- Other duties as assigned, and within the program team
- Commitment to CHOOSE 180’s vision, mission, and values.
- Two or more years of client service, youth development, or case management experience.
- Strong relationship-building and youth engagement skills.
- Experience working with or within the criminal legal system preferred.
- Excellent communication (verbal, written, and interpersonal) and facilitation skills.
- Culturally responsive and skilled at working across diverse communities.
- Adaptable and results-oriented with the ability to navigate changing priorities.
- Must have a valid WA driver’s license, insurance, and reliable transportation.
- Must be able to pass a background check and meet school clearance requirements.
- Bilingual/multilingual candidates encouraged to apply.
- Flexible schedule, including evenings and weekends as needed.
- Travel across King & Pierce County for community meetings, home visits, and events (mileage reimbursed).
- Role may include crisis intervention and de-escalation with participants and families experiencing trauma.
- Hourly rate: $33.66 per hour (approximately $70,000.00 annually)
