Coordination of health screenings, organization of health files, and supporting the Center Operating staff with health and nutrition related activities, training and curriculum to enhance the program at California Young World.
Duties And Responsibilities
Individual Responsibilities:
Monthly
- Submit the Food Program Claim to Child Nutrition (CNIPS), and submit a copy to the Accounting Dept.
- Work in collaboration with the Enrollment Coordinator (EC) to anticipate Data being requested from CYW, and state agencies, and provide necessary information for compliance. Provide support to the EC for tracking data of Alumni and current families (ethnicity, parent profiles etc.)
- Complete the Agency Annual CNIPS Application by August 31st each year to ensure certification of the CACFP Sponsor Status of CYW.
- Conduct the Annual USDA Certification of all families in the CA Food Program every October 1-31.
- Act as Liaison with CACFP DSS staff. Prepare and facilitate the annual self-review and State Administrative Review every three years
- Conduct visits to the three centers to conduct Compliance Monitoring Inspections and complete the CACFP Report 3 X per year/per site.
- Coordinate the annual Health Screenings with Healthier Kids Foundation for children 0-5 enrolled at CYW. (Dental, Vision, Auditory and Mental Health).
- Identify data and work with stakeholders (HKF, ASQ, LG, DRDP) to provide information about the status of follow up health needs for children requiring a glasses, dental work, hearing and auditory treatment, and mental health services.
- Attend the Annual CACFP National Conference – to stay informed of the latest mandates and health and safety requirements.
- Review current policies (and revise when necessary) the protocols for Daily Health Inspections, Medication Administration, the Exclusion Policy, and children who have medically involved disabilities (Asthma Albuterol Treatments, Colostomy bags, Epi Pen labels, etc.)
- Complete the “Mandatory CACFP Training” as the Food Program representative for CYW.
- Collaborate with the PD to attend all IEP meetings, or Student Study Team as requested by the Sunnyvale School District, the child’s teacher, or parents.
- Provide or coordinate training to staff on health and nutrition topics throughout the year. (CPR, dental awareness, nutrition program mandates).
- Work with HR Manager for any Impact Suites intervention services for employees and their family. (Employee Assistance Provider EAP).
- Work with HR Manager and other members of the Leadership Team to support a Work-Place Wellness Program at CYW for employees. This may include Meditation, Self-care strategies, Walking/exercise, nutritional wellness, Visualization, and/or fitness classes.
- Provide coaching and support to staff who are working with children who may demonstrate challenging behaviors including children with special needs. Refer to FIRST 5 or other providers as needed. (For example, Kids Connection – Medical Eligible & Personal Insurance for non-Medical).
- Provide oversight of the Summer Lunch Program for School Age children who have no access to healthy meals when school is out for the summer and school breaks.
- Inspect health files of staff and children to ensure compliance with mandates (TB test, physical, etc.).
- Serve as an active participant on the CYW Leadership Team.
- Liaison with F5, Innovations Collaborative, Healthier Kids Foundation, USDA and CNIPS/CACFP, other providers as assigned.
- Onboard new employees in regards to Health and Nutrition related policies and procedures.
- In collaboration with the PD and site leaders, develops, maintains, and monitors individual child health plans (e.g., medical, allergy, asthma, seizure, specialized healthcare procedures) in accordance with Title 5 (CSPP/CCTR) and Title 22 requirements, ensuring all plans meet state-mandated documentation, parent consent, and physician authorization standards.
- Coordinates with families, licensed medical providers, and program staff to gather required health information, secure medical statements, and ensure that all health plans reflect current diagnoses, medications, and care instructions.
- Ensures timely review and renewal of all health plans, including annual updates, medication expiration tracking, and verification of physician orders, per CSPP Health & Social Services Requirements (5 CCR
- 17700–17732) and CCTR program standards.
- Trains and supports teaching and site staff on the implementation of individualized health plans, emergency procedures, medication administration, universal precautions, and required documentation practices.
- Monitors classroom and site compliance with health plan implementation, medication storage, emergency equipment readiness, and required postings; conducts regular audits to ensure alignment with Title 22 Division 12 and CSPP Program Requirements.
- Serves as the primary liaison with families regarding health needs, ensuring culturally responsive communication, timely follow-up, and support in accessing health, dental, mental health, and nutrition services as required under CSPP Health & Social Services (5 CCR
- 17710–17714).
- Maintains accurate, confidential health records in compliance with state regulations, licensing requirements, and agency policy, including documentation of screenings, referrals, follow-up, and parent notifications.
- Coordinates required health screenings (vision, hearing, dental, developmental, nutrition) and ensures follow-up services are documented and completed within required timelines for CSPP and CCTR programs.
- Develops and implements nutrition plans for children with dietary restrictions, allergies, or medical conditions, ensuring compliance with CACFP, physician orders, and CSPP/CCTR program requirements.
- Collaborates with site leadership and the Executive Director to ensure program wide health and nutrition practices meet all regulatory requirements and support the wellbeing and inclusion of all children.
- Leads health related emergency preparedness efforts, including staff training, emergency medication protocols, and ensuring readiness for licensing inspections and CSPP contract monitoring reviews.
