Seeking a full-time Hospice RN Case Manager in Clinton Township, MI, to provide compassionate, patient-centered care in a home setting.
Responsibilities include delivering high-quality nursing care, performing assessments, creating care plans, monitoring patient conditions, collaborating with healthcare teams, educating families, and ensuring regulatory compliance.
Requirements include an RN license in Michigan, a minimum of 1 year of nursing experience (preferably hospice or home health), reliable transportation, and physical ability for home visits.
